New Patients

Dr. Shum requires a referral from your doctor or a primary care provider before you are seen as a new patient. 

 

Once a referral has been made, the office will call to schedule an appointment. Urgent referrals and cancer patients are seen as soon as possible. The office will notify you of the location of your appointment. Depending upon the type of consultation required, your visit may be in the hospital at the level 1 Ambulatory Care Unit (ACU) or in the office at Suite 3-22B, 885 Regent Street.

 

Please be aware of your appointment location.

 

Patients in wheelchairs or who require ambulatory assistance will be seen in the hospital at the level 1 ACU. Please inform the office if you have mobility concerns.

 

On your appointment day, please arrive at least 10 minutes prior to your scheduled time and bring the following items:

  • a complete list of medications

  • any medical records given to you by your primary doctor

  • insurance forms that need to be completed

  • a valid health card

 

Northern Health Travel Grant

Patients traveling to Sudbury to see Dr. Shum may qualify for reimbursement from the Ministry of Health and Long-Term Care. Click here for additional information, then download and complete the Northern Health Travel Grant

 

Accommodations

A number of hotels are close to the hospital and to the office. Discounted rates are available for patients traveling to Sudbury for surgery and medical care. The Travelway Inn is the closest hotel to the hospital.

Zero Tolerance Policy

Patients who exhibit belligerent, fraudulent, aggressive or profane behavior towards Dr. Shum or the office staff will have their physician-patient relationship terminated immediately in accordance with the policies set forth by the College of Physicians and Surgeons of Ontario.
